All
adult
volunteers are required to fill out a form that is used for a background check
with the Illinois State Police. This form must be submitted to the DYBA
Board before an adult can be a manager or coach.

money must comply with the Conflict of Interest Policy and file a signed
statement.

Incident Report Form must be filed after a serious injury or damage to
property at a DYBA activity.
