As the parent or guardian of the child listed above, I give permission for my child to participate in DYBSA activities.

Parent Code of Conduct for House and Travel Baseball and Softball

All parents and guardians of individuals participating in Deerfield Youth Baseball and Softball Association activities are required to acknowledge and agree to the Parent Code of Conduct as a prerequisite for participation.

1. No parent or spectator may engage in or encourage any child, or any other person to engage in unsportsmanlike conduct with any coach, parent, player, participant, official or any other attendee.

2. No parent or spectator may engage in, encourage any child, or any other person to engage in any behavior which would endanger the health, safety or well-being of any coach, parent, player, participant, official or any other attendee.

3. No parent or spectator will engage in, permit any child, or encourage any other person to use drugs, alcohol, or tobacco while at a Deerfield Youth Baseball and Softball Association event and no person may attend, coach, officiate or participate in a Deerfield Youth Baseball Association event while under the influence of drugs, alcohol, or tobacco.

4. No parent or spectator may engage in or encourage any child, or any other person to engage in verbal or physical threats or abuse aimed at any coach, parent, player, participant, official or any other attendee.

5. I understand and acknowledge that I am responsible for the actions of my guests and those of guests of my child at any Deerfield Youth Baseball and Softball Association event and that guests are subject to this same Parent Code of Conduct.

As a parent or guardian of a participant in the Deerfield Youth Baseball and Softball Association I understand and agree that if I fail to conform my conduct and the conduct of any guests associated with me or my child to the foregoing while attending, coaching, officiating or participating in any Deerfield Youth Baseball and Softball Association activity, I, my guest, and/or my child will be subject to disciplinary action, including but not limited to ejection, suspension(s) or other discipline in accord with the Deerfield Youth Baseball Association Rulebook and available under the law.

I hereby agree to abide by and support this right and to subscribe to the DYBSA Code of Conduct

 

DYBSA RETURN TO PLAY WAIVER

In consideration of being allowed to participate in any way in the DEERFIELD YOUTH BASEBALL AND SOFTBALL ASSOCIATION (DYBSA) athletics/sports program, related events and activities, the undersigned acknowledges, appreciates, and agrees that:

1. I acknowledge that there is risk of injury and/or illness from the activities involved in the program, including the potential for permanent paralysis and death, and while rules, equipment, and personal discipline may reduce the risk, the risk of serious injury does exist;

2. The risk to have contact with individuals, who have been exposed to and/or have been diagnosed with one or more communicable diseases, or other medical conditions, diseases, or maladies does exist, and it is impossible to eliminate the risk that I could be exposed to and/or become infected through contact with or close proximity with an individual with a communicable disease;

3. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others and assume all full responsibility for my participation;

4. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest DYBSA Coach, Manager, Board Member or game official immediately; and

5. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS DYBSA, their officers, officials, agents and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of the premises used to conduct the event ("Releasees"), WITH RESPECT TO ANY AND ALL INJURY, ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, UNLESS ARISING FROM THE GROSS NEGLIGENCE OR RECKLESSNESS OF THE RELEASEES OR OTHERWISE.

6. I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, BEFORE ACKNOWLEDGING THE CHECKBOX BELOW, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY AGREEING TO IT ON MY OWN BEHALF OR ON BEHALF OF THE YOUTH PARTICIPANT ASSOCIATED WITH THIS GUARDIAN ACCOUNT, AND I SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT. ACKNOWLEDGEMENT BY ADULT PARTICIPANT: By acknowledging and agreeing to the checkbox below, I agree and verify the following: 1) I consent and agree to assume the risks of participation in these programs; and 2) that I specifically agree to the release as provided herein of all the Releasees, and, for myself, my heirs, assigns and next of kin, I release and agree to indemnify the Releasees from any and all liabilities incident to my involvement or participation in these programs EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE. ACKNOWLEDGMENT BY PARENTS AND/OR LEGAL GUARDIANS OF YOUTH PARTICIPANTS: By acknowledging and agreeing to the check box below, I agree to and verify the following: 1) I am the parent or legal guardian for the youth participant associated with this guardian account, 2) that the date of birth of the youth participant associated with this guardian account is correct, 3) that as parent/legal guardian with legal responsibility for this youth participant, I consent and agree to assume the risks of his/her participation in these programs; and 4) that I specifically agree to his/her release as provided herein of all the Releasees, and, for myself, my heirs, assigns and next of kin, I release and agree to indemnify the Releasees from any and all liabilities incident to this youth participant's involvement or participation in these programs as provided above EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE

I acknowledge that I have read, and do hereby accept the terms and conditions contained in the DYBSA RETURN TO PLAY WAIVER above

 


Consent and Release to use Images

Grant, Assignment, Release and Waiver

As the parent or legal guardian of the child being registered, I hereby grant to Deerfield Youth Baseball and Softball Association and its agents and representatives (collectively, “DYBA”), the irrevocable, unrestricted right to use, publish, display, broadcast, edit, modify and distribute materials bearing such child’s name, voice, image, likeness, statements attributable to the child, and/or any other identifiable representations of the child (collectively, ”The Likeness”) in connection with or relating in any way to DYBA. These materials may appear in any form, style, color or medium whatsoever now known or later developed (including, without limitation, photographs, videotapes, films, sound recordings, software, drawings, prints, broadcast, internet and electronic media).

I agree that all materials containing The Likeness in connection with DYBA shall be and remain the sole and exclusive property of DYBA, and I hereby assign any right I may have acquired in or to such materials to DYBA. I hereby release and forever discharge DYBA from any and all claims, liabilities and damages relating to the use of The Likeness. I hereby waive any right I may have to inspect or approve the finished materials or any part or element thereof that incorporates The Likeness in connection with DYBA.

I agree and acknowledge that I have received good, valuable and sufficient consideration in exchange for the above (including, without limitation, the opportunity given to me by DYBA to use, access, read and download content appearing on the DYBA website, subject to the terms of use thereof). I acknowledge that I have fully read and understand this Grant, Assignment, Release and Waiver and that I have had any questions regarding its effect, or the meaning of its terms answered to my satisfaction. I certify that I am at least 18 years of age.

I represent that I am the parent or legal guardian of the child being registered for DYBSA, who is a minor. I understand the above and consent to the use of his/her Likeness as set forth above.


Teammate Requests

RULES FOR DYBA TEAM SELECTION

The DYBA team selection process is designed to create even teams in each division and attempt to create the most competitive and fun balance for all players involved. Every effort is taken to eliminate the possibility of unbalanced teams. For those reasons, DYBA does not allow friendship requests for its Baseball or Softball teams.  Requests for placement of a child on a team with another child, a specific coach outside of their own parent/guardian, and/or a carpool group will not be considered.

            Simultaneously, acknowledging that parent volunteers are the backbone of the DYBA organization, parent volunteer coaches may request to coach their own child alongside up to one (1) other coach under the following guidelines:

  1. All volunteers who would like to coach must be registered as a Coach/Assistant Coach/Manager in the DYBA registration system and must pass the background check process.
  2. For any coach pairing request to be considered, each of the Coaches/Managers must also request each other to ensure the coach requests are mutual. For Example: If Coach Smith requests to be paired with Coach Jones, then Coach Jones must also request to be paired with Coach Smith.
  3. No more than two (2) coach volunteers can request to be placed on the same team with each other.

DYBA will then make every effort to place the requesting coaches on the same team.  Please note that while every effort will be made, this is not a guarantee.

All siblings/relatives within the same age group will automatically be placed on the same team unless otherwise directed by their parents/guardians.

To request that coaches be placed on the same team, please follow these guidelines. Coach requests are only accepted in the comments section of the coach registration system once registration for that specific league opens. No phone or email requests will be accepted. All correctly submitted coach pairing requests will be recorded for consideration, but, as always, not guaranteed. The DYBA League Board and each of the Baseball/Softball Directors reserve all final decisions on teams to accommodate extenuating circumstances as may be needed.
